Pregnancy and breast feeding

If you are pregnant or breast-feeding, think you may be pregnant or are planning to have a baby, ask your doctor or pharmacist for advice before taking this medicine.

Pregnancy

It is very important to stop smoking during pregnancy as it can slow down the growth of your baby. It can also lead to premature labor or late fetal death. It is advisable to stop smoking without using drugs containing nicotine.

If you cannot quit smoking without nicotine replacement therapy, NIQUITIN MENTHE GLACIALE can be used after consulting the healthcare professional who is monitoring your pregnancy, your family doctor, or a doctor from a center specializing in help. for people wishing to quit smoking.

Feeding with milk

If you smoke a lot and do not plan to quit, you should not breastfeed your baby, but resort to artificial breastfeeding.

If you want to stop smoking and breastfeed your child, seek the advice of your doctor or a doctor at a center specializing in smoking cessation.

This is because the use of this medicine should be avoided during breast-feeding because nicotine passes into breast milk and can affect your baby. If your doctor has recommended you to use NIQUITIN GLACIAL MINT, the dose should be taken immediately after a feed or more than 2 hours before the next feed.

In addition, due to the passage into milk of the terpene derivatives (mint / eucalyptus flavoring) contained in this drug and the potential neurological toxicity in infants, you should consider another chewing gum flavor.

Dosage

Chewing gum dosed at 4 mg are more particularly suitable for smokers who are heavily or very heavily dependent on nicotine (Fagerström score of 7 to 10 or smoking more than 20 cigarettes per day: see "Health education advice" ).

Complete smoking cessation

Treatment for nicotine addiction is usually done in 2 phases.

1st phase

Chew a gum whenever the urge to smoke appears.

The number of chewing gums dosed at 4 mg is generally 8 to 12 gums per day and should in no case exceed 15 gums per day.

The duration of this phase is approximately 3 months, but may vary depending on the individual response.

2nd phase

When the urge to smoke is completely overcome, gradually reduce the number of gums chewed per day.

The treatment will be stopped when the consumption is reduced to 1 to 2 gums per day.

It is recommended not to use the chewing gum beyond 12 months.

If during your treatment you continue to have an irrepressible urge to smoke or if your treatment has failed, CONSULT YOUR DOCTOR.

Temporary abstinence from tobacco

Chew a gum whenever the urge to smoke appears.

The number of chewing gums dosed at 4 mg is generally 8 to 12 gums per day and should in no case exceed 15 gums per day.

Reduction in tobacco consumption

Whenever possible, alternate nicotine gums and cigarettes.

Chew a gum as soon as you feel the urge to smoke in order to minimize cigarette consumption and stay as long as possible without smoking.

The number of chewing gums per day is variable and depends on your needs. However, it should not exceed 15 gums per day.

If a reduction in cigarette consumption has not been obtained after 6 weeks of treatment, it is recommended to seek advice from a healthcare professional.

Reducing tobacco consumption should gradually lead you to stop smoking altogether. This stopping should be attempted as soon as possible within 6 months of the start of treatment. If more than 9 months after the start of treatment, the attempt to stop smoking has failed, it is recommended to seek advice from a health professional.

It is recommended not to use the gums beyond 12 months.

To increase the chances of success in quitting smoking completely, it is important to aim for a reduction of more than 50% in consumption during this phase of reduction. NIQUITIN MENTHE GLACIALE chewing gums help achieve this goal before you quit smoking.

Method and route of administration

Oral route.

To be active, the nicotine contained in the gum and released by chewing, must be absorbed through the oral mucosa.

If nicotine is swallowed with saliva, it has no effect. It is therefore important not to chew the gum too vigorously.

Thus, it is advisable to chew the gum for the first time, then keep it against the cheek for about 10 minutes: the gum softens and allows the nicotine to be released gradually, to be absorbed by the oral mucosa, thus passing into the circulation. .

Then chew the gum very slowly (about 20 times in 20 minutes).

The chewing gum should be kept in the mouth for about 30 minutes. After 30 minutes, the chewing gum will no longer deliver nicotine.

Duration of treatment

For complete cessation of smoking:

The duration of use is limited to 12 months.

If the feeling of withdrawal persists or if it is difficult to stop this medication, consult your doctor.

If you take more NIQUITIN GLACIAL MINT 4 mg WITHOUT SUGAR, medicated chewing gum sweetened with sorbitol and xylitol than you should

Immediately consult your doctor or pharmacist.

In case of accidental overdose or if a child swallows one or more chewing gum: CONTACT A DOCTOR OR EMERGENCY SERVICE.

Severe poisoning can occur and is manifested by hypotension, weak and irregular heartbeat, difficulty in breathing, prostration, cardiovascular collapse and convulsions.

If you forget to take NIQUITIN GLACIAL MINT 4 mg WITHOUT SUGAR, medicated chewing gum sweetened with sorbitol and xylitol

Do not take a double dose to make up for the dose you forgot to take .

If you stop taking NIQUITIN GLACIAL MINT 4 mg WITHOUT SUGAR, medicated chewing gum sweetened with sorbitol and xylitol

Not applicable.

Other

Health Education Council:

Stopping smoking as soon as possible has immediate health benefits. It is never too late to quit smoking even if you have smoked a long time and a lot.

As soon as you stop smoking:

· You will no longer absorb the irritating and toxic substances of cigarettes,

· You may notice certain improvements: gradual disappearance of headaches, cough, irritation of the throat, shortness of breath,

· You will often have the satisfaction of finding better sleep, the flavor of food, smells and improving your physical performance,

· You will lower your risk of developing lung cancer or cardiovascular disease.

Moreover, in heart or arterial diseases (angina pectoris, arteritis of the lower limbs…), it is particularly imperative to succeed in quitting smoking.

What is tobacco addiction?

Tobacco addiction is twofold:

· Psychological dependence:

Smoking a cigarette involves a ritual of automatic and repetitive gestures associated with particular circumstances (pleasure, stress response, intellectual stimulation, support for morale, need for the gesture of conviviality). It is about psychological and behavioral addiction.

· Physical dependence:

The body's need for nicotine is due to the effects of this substance on the nervous system. It is about physical dependence. Non-satisfaction of this need leads to a feeling of lack (see description in the chapter " How to overcome your physical dependence ?").

Nicotine substitutes aim to treat this addiction.

How to overcome your psychological dependence?

· Stopping smoking completely is necessary to increase your chances of success, but in addition:

o Choose a D day in the period which seems most favorable to you.

o Throw away your last pack of cigarettes, your lighter and put away your ashtrays.

o In a couple of smokers, it is preferable that the attempt to quit be made at the same time by both spouses.

· Tell those around you and co-workers that you are quitting. Ask them to respect your decision (not to smoke near you, not to offer you any more cigarettes).

· Prepare to face the urge to smoke. It can occur suddenly and intensely, especially during the first few weeks after quitting smoking, but does not last more than 3 or 4 minutes.

You must plan what you will do in this case and act very quickly so as not to "crack", for example:

o drink a large glass of water,

o to chew gum,

o count to 100,

o take a few deep breaths,

o leave the room,

o change activity ...

· Avoid situations associated with taking cigarettes (coffee, aperitif ...), avoid temptations (sit with non-smokers).

· Find compensation for smoking (leisure, sport, cultural activity, etc.).

How to overcome your physical addiction?

If you are addicted to nicotine, the success of your process may require the use of nicotine replacement therapy.

In order to assess your level of dependence, the Fagerström test is offered to you.

FAGERSTROM TEST

How long after you wake up do you smoke your first cigarette?

· within 5 minutes: 3

· 6 to 30 minutes: 2

· from 31 to 60 minutes: 1

· after 60 minutes: 0

Do you find it difficult not to smoke in places where it is prohibited?

· yes: 1

· no: 0

Which cigarette of the day would it be most difficult for you to give up?

· the first: 1

· another: 0

How many cigarettes do you smoke per day?

· 10 or less: 0

· 11 to 20: 1

· 21 to 30: 2

· 31 or more: 3

Do you smoke at a faster rate in the morning than in the afternoon?

· yes: 1

· no: 0

Do you smoke when you are so sick that you have to stay in bed most of the day?

· yes: 1

· no: 0

TOTAL POINTS

score from 0 to 2 : you are not addicted to nicotine.

You can quit smoking without the need for nicotine replacement therapy.

If, however, you dread stopping, seek advice from your pharmacist or doctor.

score 3 to 4 : you are weakly addicted to nicotine.

score 5 to 6 : you are moderately addicted to nicotine.

Using nicotine replacement therapy will increase your chances of success.

Ask your pharmacist or doctor for advice so that they can help you choose the most suitable treatment for you.

score of 7 to 10 : you are strongly or very strongly dependent on nicotine.

The use of nicotine replacement therapy is recommended to help you overcome this nicotine addiction. This treatment must be used in sufficient and appropriate dose.

Take advice from your pharmacist or your doctor, possibly as part of a specialized consultation to help with smoking cessation.

Withdrawal-related disorders can persist for a long time and the duration of treatment should be sufficient, usually one to three months.

If you feel, despite treatment, a persistent feeling of "lack" such as:

· craving to smoke,

· irritability, sleep disturbances,

· restlessness or impatience,

· difficulty concentrating,

Take the advice of your doctor or pharmacist as dose adjustment may be necessary.

How can you get help?

· Support from healthcare professionals

The support, advice and follow-up that your doctor or pharmacist can provide will be invaluable during the first six months after you quit smoking. You can also resort to a specialized consultation to help with smoking cessation, especially if you have already failed.

· Psychological support

Quitting smoking requires a supportive family and social environment. If you are concerned that the difficulties are too great, psychological support or temporary treatment can help.

· Dietetics

In the months after quitting smoking, you may notice moderate weight gain. If you have any concerns in this area, you can contact a healthcare professional who will be able to advise you.

Why did you start smoking again?

Consider this relapse as a mishap, as a normal learning event: relapse is not a failure.

Analyze the reasons why you have resumed smoking.

· Because of physical dependence

This may have been incorrectly taken into account or may reappear after several months, for example during prolonged stays in smoky atmospheres.

· Because of weight gain considered excessive

You may need to change your eating habits a bit before your next attempt to quit smoking.

· Due to a decrease in motivation ,

because of a painful event or a difficult period, because of psychological difficulties which appeared on stopping smoking (depression, anxiety, etc.), because of a very tempting situation. Understand the reason for this relapse to prevent it if possible on the next attempt.

In any case, do not hesitate to seek advice from a health professional or to resort to a specialized consultation to help with smoking cessation.

If you have started smoking again, do not be discouraged, experience shows that your chances of success increase after one or more attempts to quit.

INDICATIONS (summary)
This drug helps smokers to detoxify themselves from tobacco. Chewing gum brings into the blood, via the oral mucosa , nicotine, the main active substance in tobacco, responsible for addiction.
It is used either to quit smoking permanently or to quit temporarily.
CONTRAINDICATIONS (summary)
This medication should not be used in non-smokers or occasional smokers.
ATTENTION (summary)
With a view to definitive detoxification, stopping smoking is a necessary condition for the success of the treatment.
Precautions are necessary in case of stomach ulcer or duodenum , renal or hepatic insufficiency . In these situations, a prior medical consultation is essential before using this medication; stopping smoking without substitution treatment may be necessary.
Do not use other drugs that contain nicotine, especially transdermal patches (see Interactions).
This chewing gum is a medicine. Keep it out of the reach of children.
Wearing a dental prosthesis can cause problems when chewing gums: consult your dentist.
If, after 10 days of treatment, you start to smoke again: consult your doctor in order to adopt a new anti-smoking strategy with him.
DRUG INTERACTIONS (summary)
The products contained in tobacco smoke can accelerate the rate of elimination of certain drugs, notably theophylline. When stopping smoking, these drugs may have their effect enhanced and signs of overdose may appear. Contact your doctor if there is any doubt.
PREGNANCY and BREASTFEEDING (summary)
Pregnancy :
Smoking in pregnant women can cause fetal growth retardation. Stopping smoking is therefore always desirable. In women heavily dependent on nicotine, the use of this drug may help, but the effects of nicotine alone on the fetus are not well understood. This drug has the advantage of not exposing the fetus to other toxins present in tobacco smoke. In summary: if the ideal is not to smoke and not to expose the fetus to nicotine, the use of this drug is a lesser evil compared to tobacco.

Feeding with milk :
Nicotine passes into breast milk, breastfeeding is not recommended during treatment.
If breastfeeding is nevertheless chosen, take the gums preferably after the suckling.
DIRECTIONS FOR USE AND DOSAGE (summary)
The choice of dosage depends on the initial dependence on nicotine. The 4 mg gums are suitable for highly or very heavily dependent smokers; they can also be used by smokers with little addiction who prefer to limit the number of gums chewed daily.
In practice, it is useful to know the signs of under or overdose in order to adjust the dosage after a few days of treatment:
  • overdose results in the condition of the smoker who has smoked too much and mainly results in nausea and headache. Vision or hearing disturbances, tremor, dizziness are also possible;
  • an underdose causes withdrawal symptoms : irritability, nervousness, stomach cramps (a kind of painful hunger), difficulty concentrating.
Chew each gum very slowly to allow the slow and regular absorption of nicotine by the blood vessels of the tongue: after a first chewing, keep the gum against the cheek for 10 minutes to soften it. Then chew the gum every minute (about 20 times in 20 minutes). After 30 minutes of chewing, the gum is no longer effective.

Usual dosage:

  • Adult :
    • Complete smoking cessation : for this purpose, the treatment of nicotine dependence usually follows 2 phases: for 3 months, chew a gum each time the need to smoke arises, without exceeding 30 gums of 2 mg or 15 gums at 4 mg per day. This 3 month period may vary from person to person and its end corresponds to a sharp decrease or disappearance of the urge to smoke. The second phase, which lasts several weeks, corresponds to a gradual decrease in the number of daily gums, until stopping. Treatment should not exceed 6 months in total.
    • Temporary cessation of smoking : the principle is the same, but the duration of treatment depends on the context which required a temporary cessation of smoking.
    • Reduction in the number of cigarettes : alternate between gums and cigarettes. Chew a gum as soon as you feel the urge to smoke in order to minimize cigarette consumption and stay as long as possible without smoking. Do not exceed 15 gums at 2 mg or 9 gums at 4 mg per day. If a sharp decrease in cigarette consumption has not been obtained after 6 weeks of treatment, it is recommended to seek advice from a doctor.
Do not exceed 6 months of continuous treatment.
ADVICE (summary)
Nicotine taken by way oral replacing the cigarette has the same cardiovascular harmful than that in tobacco. Only the risk of lung cancer disappears. When stopping smoking is motivated by urgent medical reasons: recent infarction , arteritis , gastritis or ulcer , it is preferable to stop the absorption of nicotine in all its forms.
Like all anti-smoking methods used to help smokers, this medication can only be effective when the desire to quit is personal and comes with a strong motivation. It is this motivation that will make it possible to fight against the urge to smoke, without being forced to constantly appeal to the will which dulls over time. A lasting cessation of tobacco in the smoker supposes that this one considers tobacco as a poison, an unacceptable dependence. He must get used to the idea that he will never smoke again, from the start of weaning. Every day without tobacco should be experienced as a liberation, and not as an ordeal. The smoker who is in this state of mind is a good candidate for permanent withdrawal.
Stopping smoking, which is a real drug, induces many problems: weight gain is frequent, concentration disorders can last for several weeks. A mild depressive state is possible. Weight gain should be combated by diet and, if possible, by a sports program which has the advantage of reconciling the smoker with his body.
Do not hesitate to consult your doctor in case of difficulty; his advice and experience will be invaluable in the fight against relapses.
POSSIBLE SIDE EFFECTS (summary)
Irritation of the mouth or throat, pain in the jaws.
Nausea, digestion disturbances, hiccups, headache, palpitations , rash .
Rarely: heart rhythm disturbances , allergic reaction .
Like all medicines and sweets containing non-absorbable sugars (sorbitol, maltitol ...), possibility of diarrhea in case of high intake.

GLOSSARY (summary)

arteritis
Disease of the arteries favored by smoking and most often localized to the legs. It is characterized by thickening of the arterial walls which interferes with blood circulation and causes pain when walking (intermittent claudication). Arteritis of the lower limbs increases the risk of myocardial infarction and requires drug treatment. At an advanced stage, it can be treated surgically.
Synonym: Obliterating arterial disease of the lower limbs.

diarrhea
The strict medical meaning of diarrhea is "passing too much and too much stool". In fact, common sense equates diarrhea with the notion of frequent, watery stools. Normally, the stools are pasty, but the emission of watery or barely formed stools, without pain or particular associated disorder, is not pathological. We can speak of diarrhea when the emission of liquid stools is repeated throughout the day, and the needs are urgent or painful.
Many medicines can speed up bowel movements and make stools more watery, without this side effect causing any real concern.
Antibiotics can alter the digestive flora, essential for digestion, and cause diarrhea that is more or less embarrassing but benign. The effect appears immediately or after a few days of treatment. A severe and exceptional form of diarrhea, pseudomembranous colitis, can be observed after antibiotic treatment; this condition results in the emission of mucus and false membranes (resembling scraps of skin) associated with abdominal pain; constipation can replace the initial diarrhea. Pseudomembranous colitis can occur several days after stopping antibiotic treatment and requires urgent medical advice.

transdermal patches
System, sometimes called a patch or patch, allowing the absorption of a drug through the skin: it ensures the diffusion of the active substance towards the blood vessels of the dermis. The substance then travels through the blood and can act as if it had been swallowed or injected. This device avoids the hepatic first pass effect.

duodenum
Part of the intestine into which food from the stomach enters.

skin rash
Appearance of pimples or plaques on the skin. These lesions can be due to a food or a drug, and reflect an allergy or a toxic effect. Many viruses can also cause rashes of pimples: those of rubella, roseola and measles are the best known.

depressed state
State of moral suffering associating demotivation, fatigue, a feeling of uselessness, self-deprecation. If left untreated, the depressive state may progress to severe depression with feelings of incurability, guilt, and a risk of suicide.

gastritis
Inflammation of the stomach favored by stress, tobacco, alcohol and certain drugs such as aspirin or anti-inflammatory drugs.

infarction
Destruction of part of the heart muscle (myocardium), deprived of blood by obstruction of its arteries.

hepatic
Inability of the liver to perform its function, which is essentially the elimination of certain waste, but also the synthesis of many biological substances essential to the body: albumin, cholesterol and coagulation factors (vitamin K, etc.).

renal failure
Inability of the kidneys to eliminate wastes or drugs. Advanced kidney disease does not necessarily result in a decrease in the amount of urine passed. Only a blood test and the determination of creatinine can reveal this disease.

mucous
Tissue (membrane) that lines the cavities and ducts of the body communicating with the outside (digestive tract, respiratory system, urinary tract, etc.).

palpitations
Abnormal perception of irregular heartbeat.

dosage
Quantity and distribution of the dose of a drug according to the age, weight and general condition of the patient.

allergic reaction
Reaction due to the body's hypersensitivity to a drug. Allergic reactions can take on a wide variety of aspects: hives, angioedema, eczema, rash reminiscent of measles, etc. Anaphylactic shock is a generalized allergic reaction that causes discomfort due to a sudden drop in blood pressure.

overdose
Taking too much of a drug exposes the intensity of the side effects to an increase, or even the appearance of specific side effects.
This overdose can result from accidental poisoning, or intentional with the aim of suicide: it is then advisable to consult the poison control center of your region (list in appendix of the book). But most often, the overdose is the consequence of an error in understanding the prescription, or of the search for an increase in effectiveness by exceeding the recommended dosage. Finally, untimely self-medication can lead to excessive absorption of the same substance contained in different drugs. Certain drugs are more particularly exposed to this risk, because they are considered (wrongly) as harmless: vitamins A and D, aspirin, etc. Stopping or reducing drug intake can eliminate the disorders associated with an overdose.

symptoms
Disturbance experienced by a person with an illness. The same symptom can represent different diseases and the same disease does not necessarily give the same symptoms in all patients.

heart rhythm disturbances
Serious or mild abnormality in the frequency of heart contractions. Extrasystole is a contraction that occurs just before or after a normal contraction, often seen as a slight shock in the chest. Fibrillation is an irregular, disorderly contraction. Other disorders exist: torsades de pointes, Wolf-Parkinson-White syndrome, Bouveret's disease, tachysystole, flutter and atrioventricular block, etc.

ulcer
Hollow lesion of the skin, mucous membranes or cornea.
  • Leg ulcer: chronic wound caused by poor blood circulation.
  • Peptic ulcer: localized wound in the lining of the stomach or duodenum, due to excess acidity and very often to the presence of bacteria (Helicobacter pylori). The ulcer is favored by stress, alcohol, smoking and taking certain medications (aspirin, NSAIDs, etc.).

way
  • Path (route of administration) used to administer the drugs: oral, sublingual, subcutaneous, intramuscular, intravenous, intradermal, transdermal.
  • Set of hollow organs allowing the passage of air (respiratory tract), food (digestive tract), urine (urinary tract), bile (bile duct), etc.
