Focus on the prostate
The prostate is a chestnut-shaped organ made up of dozens of small, branched glands. It surrounds the upper (prostatic) part of the urethra, which lies directly beneath the bladder. The prostate secretes a milky, slightly alkaline fluid that makes up a third of the volume of spermatic fluid and plays a role in sperm activation. This fluid enters the urethra through several small ducts, thanks to the contractions of the fibrous capsule of the prostate and its ramifications, at the moment of ejaculation.
As the prostate is located directly in front of the rectum, its volume and texture can be assessed by palpating it through the anterior wall of the rectum with a finger inserted into the rectum. Prostate hyperplasia affects almost all older men, causing constriction of the prostatic part of the urethra. This makes urination difficult, and increases the risk of inflammation of the bladder (cystitis), kidneys (pyelonephritis) or prostate itself (prostatitis).
There is a classic surgical treatment for the latter: prostatectomy, but other means also exist:
- the use of drugs: antiandrogens (finasteride), which shrink the prostate; or alpha-blockers, which reduce the tone of prostate muscle fibers.
- balloon treatment, which involves inserting a balloon into the prostatic part of the urethra and inflating it to push back the prostatic tissues that are exerting pressure.
- urethral insertion of a fine needle that emits low-frequency radiation (ultrasound) or laser radiation to burn away excess prostate tissue.
Inflammation of the prostate is the main reason why men consult a urologist. Prostate cancer is the second leading cause of cancer death in Canadian men.
Several problems can affect the prostate. The most common are :
- Prostatitis, or inflammation of the prostate gland
- Benign prostatic hypertrophy or prostatic adenoma, or enlargement of the prostate gland, which is very common in older men.
- Prostate cancer
