The severity of an anklespraindepends on the nature and extent of the injuries to the ankle joint. In the mildest cases, the lateral ligaments have only been slightly stretched. If there is no swelling, if the pain is mild and short-lived, and if there is no functional impairment, then it can be considered asimple sprain. The situation is more serious if the ligaments tear or rupture and if other nearby structures are affected (tear of the joint capsule or damage to bone structures in the ankle, foot, or leg).

The general principle for promoting healing of the ankle ligaments is to eliminate all tension on the ankle. Since no stress is observed when bearing weight while walking or during ankle flexion and extension movements, a removableankle bracecan be prescribed for the patient. This brace is placed inside the shoe, allows for full-weight-bearing walking, and neutralizeslateral movements. It can be removed at night and while sitting. The classic elastic strapping bandage serves the same purpose but is less convenient to use.
The other components of treatment are non-specific and follow general trauma guidelines: icing, elevation of the limb, anti-edema compression, anti-inflammatory medications after the third day, analgesics, etc.
In the case ofa severe sprain—especially if there is a ligament tear—it is prudent to apply a cast for three weeks, followed by three weeks of splinting.
