Rectal laxatives (also called contact laxatives) are recommended as a first-line treatment in cases of constipation of rectal origin (or distal constipation which results in a feeling of anal obstruction, incomplete evacuation, pushing efforts) . These laxatives cause stool to be expelled by contraction of the rectum. They should not be used for a long time, as they may interfere with the normal contraction reflex of the rectum which allows stool to pass.
Rectal laxative drugs (Eductyl, Glycerin Suppositories, Microlax) contain glycerin or sodium bicarbonate. With 5 to 15 minutes of onset of action to relieve constipation, they are instant remedies. These laxatives are only effective in case of terminal constipation, responsible for an accumulation of stool in the rectum. Taken over the long term, these laxatives cause a loss of the exemption reflex, that is to say of the natural stimulation that pushes to go to the toilet to evacuate the stool.
Microlax is a drug indicated for the symptomatic treatment of low constipation , especially rectal dyschezia. The Microlax range of drugs originated from Johnson & Johnson laboratories. It includes in particular rectal gels indicated in the treatment of temporary constipation. Rectal laxatives are widely used to treat constipation, either by self-medication or by prescription. Dyschesia and certain continence disorders are the main indications for rectal laxatives. Laxative suppositories or enemas may be given.
Laxatives intended for the rectal route, available in France, differ in their chemical formula and mode of action. They are classified according to their mode of action.
Osmotic effect
Microlax® is a rectal gel based on sorbitol and sodium lauryl sulfate. It is an osmotic laxative which acts in 5 to 20 minutes according to its SPC (summary of product characteristics).
Glycerol (alias glycerin), in the form of suppositories or gel (such as Bébégel®, which can also be used in adults despite its name), acts in 5 to 30 minutes according to its SPC (Vidal 2008); its action undoubtedly results from an effect which is at the same time osmotic, stimulating and lubricating. This is the most widely used active ingredient. The long-term repetition of its administration is probably an element of disturbance of the normal reflex.
In some preparations, several active ingredients can be combined. This is the case of Rectopanbiline® rectal gel and Rectopanbiline® suppository which contain in particular beef bile extract (triggers the exemption reflex) and glycerin.
Stimulating effect
Bisacodyl (Dulcolax®) is a stimulant laxative, which works in 30 to 60 minutes. This form is reserved for adults, at the rate of one suppository per day, half an hour before the time chosen for intestinal evacuation.
Normacol® enema is composed of sodium phosphates in hypertonic solution. According to its SPC, it is an “irritant” laxative which acts within 5 minutes, contraindicated in the event of hydro-electrolyte disorders with sodium retention.
Gas release, CO2
Eductyl® suppositories consist of potassium acid tartrate and sodium bicarbonate which release carbon dioxide in a humid environment in the rectum. According to their SPC, the volume of carbon dioxide released increases intrarectal pressure, which triggers the exoneration reflex in a few minutes. They exhibit good tolerance.
