2. BEFORE YOU USE NICORETTESKIN 15 mg / 16 hours, transdermal patch?

Never use NICORETTESKIN 15 mg / 16 hours, transdermal patch:

· if you are allergic to nicotine or any of the other ingredients of this medicine, listed in section 6.

· if you do not smoke or if you are an occasional smoker,

· if you have a skin condition that may interfere with the use of a transdermal system.

Dosage

Three sizes of transdermal devices are available: 9; 13.5; 22.5 cm 2 , corresponding to 3 different dosages: 10 mg / 16 hours, 15 mg / 16 hours, 25 mg / 16 hours .

The transdermal patch should be applied to a healthy part of the skin in the morning at dawn, and removed at bedtime by h later. The treatment mimics the fluctuations of nicotine during the day in the smoker, without administration of nicotine at night. Daytime nicotine administration by the transdermal patch does not cause sleep disturbances seen during overnight nicotine administration.

· It is recommended for heavily dependent smokers (Fagerström test score ≥ 7) to start with the transdermal patch 25 mg / 16 hours. A transdermal patch will be applied daily in the morning and removed at night for 8 weeks.

· Dose reduction is then initiated. A transdermal patch of 15 mg / 16 hours is used daily for 2 weeks and then a transdermal patch 10 mg / 16 hours during also 2 weeks.

· It is recommended for moderately addicted smokers (Fagerström test score 5 and 6), to start with step 2 with the transdermal patch 15 mg / 16 hours.

o If smoking cessation is obtained the first week with dosing 15 mg / 16 hours, a transdermal patch will be applied daily in the morning and removed in the evening, for 8 weeks then a transdermal patch of 10 mg / 16 hours will be applied during 4 weeks.

The entire treatment should last at least 3 month.

The duration of the treatment may however vary depending on the individual response.

It is recommended not to use transdermal patches beyond 6 month. Some "ex-smokers" may, however, require a longer duration of treatment to avoid relapse.

In case of combination of NICORETTESKIN with an oral substitution form, the dosage to follow is the same as when these drugs are used alone in monotherapy. For the dosage of the oral form, consult the package leaflet of the chosen drug.

In case of association, use only one oral form at a time over a period of 24 hours.

Administration mode

· Wash your hands before applying the transdermal patch.

· Open the bag with a pair of scissors and take out the patch.

· Apply the patch immediately after opening on a dry skin surface, without skin lesions and where hair is scarce (hip, upper arm or chest).

· Remove as much of the aluminum foil as possible. Avoid as much as possible touching the sticky surface of the transdermal patch with your fingers.

· Apply the sticky part with your fingers and remove the remaining part of aluminum foil.

· Press the transdermal patch firmly on the skin with your fingers.

· Rub firmly with your fingers on the edges so that the patch sticks properly.

· The patch should be applied in the morning and removed at bedtime, approximately 16 hours later.

· The application site should be changed daily avoiding, if possible, replacing the patch at the same place for a few days.

· It is essential to fold the transdermal patch on itself after use before disposing of it in a safe place.

· Any transdermal patch used or not should be kept out of the reach of children.

· During handling, avoid contact with eyes, nose and wash hands after application.

· Do not cut the transdermal patch.

· In case of sea bath or extended pool, you have 2 possibilities :

o Before bathing, remove the patch and replace immediately on its support. It can be glued on a well dried skin.

o Cover the transdermal patch with a waterproof adhesive bandage for the duration of the bath.

· In the case of a short shower, the transdermal patch can be held in place by avoiding directing the shower jet over it.

4. WHAT ARE THE POSSIBLE SIDE EFFECTS?

Like all medicines, this medicine can cause side effects, although not everybody gets them.

Very common: affects more than 10 users in 100

- itching (pruritus).

Frequent: affects 1 to 10 users in 100

- rashes and hives,

- nausea and vomiting,

- digestive disorders,

- redness of the skin,

- headache,

- dizziness.

Uncommon: affects 1 to 10 users in 1000

- palpitations (unusual feeling to your heartbeat), or rapid heartbeat (tachycardia),

- abnormal dreams,

- excessive sweating, hot flashes,

- fatigue, malaise, pain / discomfort in the chest, difficult breathing (dyspnea),

- reaction to the application site,

- allergic reactions,

- hypertension,

- pain in the muscles,

- tingling sensation that can spread around the application site.

- Heart rhythm disorders.

INDICATIONS (summary)
This medication helps smokers to detoxify themselves from tobacco. The transdermal patch (patch) contains nicotine, the main active ingredient in tobacco; it passes through the skin, gains blood and acts on the nervous system, thus allowing the smoker not to feel the symptoms due to stopping smoking.
CONTRAINDICATIONS (summary)
This medicine should not be used in the following cases:

ATTENTION (summary)
In the context of a definitive detoxification, stopping smoking is a necessary condition for successful treatment.

DRUG INTERACTIONS (summary)
The products contained in tobacco smoke can accelerate the elimination rate of certain drugs, theophylline in particular. When stopping smoking, these drugs may have an increased effect and signs of overdose may occur. Contact your doctor if there is any doubt.
PREGNANCY and BREASTFEEDING (summary)
Pregnancy :
Smoking in pregnant women can cause fetal growth retardation. Stopping smoking is therefore always desirable. In women highly dependent on nicotine, the use of this drug may be helpful, but the effects of nicotine alone on the fetus are poorly understood. This drug has the advantage of not exposing the fetus to other toxins found in tobacco smoke. In summary: if the ideal is not to smoke and not expose the fetus to nicotine intake, the use of this drug is a lesser evil compared to tobacco.

Breastfeeding:
Nicotine passes into breast milk, breastfeeding is not recommended during treatment.
DIRECTIONS AND DOSAGE (summary)
There are 3 dosages corresponding to 3 different patch surfaces: the amount of nicotine that passes into the blood is proportional to the surface of the transdermal patch .
The highest dosage is for heavy smokers or smokers of nicotine-rich cigarettes; a strong nicotine addiction may also lead moderate smokers to prefer the strong dosage.
In practice, it is useful to know the signs of under or overdose to adapt the choice of dosage after 1 or 2 days of treatment:
  • Overdose results in the smoker's over-smoking status, which mainly results in nausea and headache. Disturbances of sight or hearing, tremor, vertigo are also possible;
  • under-dosing causes withdrawal symptoms: irritability, nervousness, stomach cramps (a kind of painful hunger), difficulty concentrating.
Once weaning is achieved, it is advisable to switch to a less dosed patch after a period of 8 weeks, then stopped after the use of the lowest dosage.
Put the patch on a dry area, poor in hair, and free of cutaneous lesions. Release the adhesive surface and carefully apply the stamp by applying pressure to its entire surface. Change location every day.
Remember to wash your hands after application and avoid contact with the eyes or nose.

Usual dosage:

  • Adult: the patch is placed in the morning after the toilet and removed at bedtime; do not use more than one patch per day.
Keeping the patch at night helps to combat the symptoms of lack of sunrise. Nevertheless, nighttime wear can cause sleep disturbances. That is why it is better to remove the stamp at bedtime.
It is recommended not to exceed 12 months of continuous treatment.
This medicine may be used alone or in combination with 2 mg gums or NICORETTE inhaler . In this case, follow the recommendations of your doctor.
TIPS (summary)
Nicotine taken by the dermal route to replace the cigarette has the same cardiovascular toxicity as that which is contained in tobacco. Only the risk of lung cancer disappears. When the smoking cessation is motivated by urgent medical reasons: recent infarction , arteritis , gastritis or ulcer , it is better to stop the absorption of nicotine in all its forms.
Like all smoking cessation methods used to help smokers, this medication can only be effective when the will to quit is personal and comes with a strong motivation. It is this motivation that will fight against the urge to smoke, without being forced to constantly appeal to the will that dulls over time. A sustainable smoking cessation by the smoker supposes that the smoker considers the tobacco as a poison, an unacceptable dependence. He must be convinced that he will never smoke again, from the beginning of weaning. Every day without tobacco must be lived as a liberation, and not as a nightmare. The smoker who is in this state of mind is a good candidate for permanent weaning.
The cessation of smoking, which is a real drug, causes many problems: weight gain is common, disorders of concentration can last several weeks. A slight depressive state is possible. Weight gain should be combated by a diet, and if possible by a sports program that has the advantage of reconciling the smoker with his body. transdermal patch
